Notification of Change of Head of Household, Household Separation, Merger, and Change of Composition

If there are changes to the head of the household, separation, merger, or changes in composition of household members, notification is required.

Notification Period

Within 14 days from the date of change
Note　As a general rule, the change will take effect from the date of notification.

Place of Notification

• Citizen Affairs Division, 1st Floor, City Hall
• Hirao Branch Office
• Wakabadai Branch Office (i Plaza 1st Floor)

Notice Changes to households including foreign residents cannot be processed at the Branch Office. Please visit the Citizen Affairs Division on the first floor of City Hall.

Notifier

• The person who made the change or the head of the household
• Agent with a power of attorney

Notice The power of attorney must be signed in full by the grantor.

Required only if applicable

• For foreign residents whose household composition has changed and cannot indicate their relationship with the head of the household, a certificate indicating this and a translation are required.
• Health Insurance Card
• Late-Stage Elderly Medical Care Insured Person Certificate
• Long-Term Care Insurance Insured Person Certificate
• Infants and Young Children Medical Certificate, etc.

Notes: On holiday open days, hours are from 8:30 AM to 11:45 AM and 1:00 PM to 5:00 PM
Note: Holiday openings are only at City Hall.
