The Late-stage Elderly Medical Insurance Card can be used until its expiration date.

Updated: December 2, 2024

 As a result of the transition to a system in which the My Number Card will be used as a health insurance card from December 2nd, issuance of health insurance cards will cease. If there are no changes to the eligibility information, the health insurance card you currently have can be used as before until its expiration date.
Expiration date: July 31, 2025 (Note: If the expiration date is earlier than the date stated above, the expiration date will apply.)

Notification of qualification confirmation and qualification information

Eligibility confirmation letter

 If you acquire a new qualification or your qualification information changes between December 2, 2024 and July 31, 2025, we will issue you a qualification confirmation certificate without you having to apply, regardless of whether you have a My Number Health Insurance card or not.
After that, those who do not have a My Number health insurance card will be issued a qualification confirmation certificate that can be used from August 1, 2025, scheduled to be issued around mid-July, 2025.
(Note: My Number Health Insurance Card refers to a My Number Card that has been registered as a health insurance card.)

 Even if you do not have a My Number insurance card, you can receive insured medical treatment in the same way as with an insured card by presenting the eligibility confirmation document at a medical institution.

Notification of qualification information

 Those who are enrolled in the Long-Term Care Insurance for the Elderly and have a My Number insurance card will be notified of their eligibility information around mid-July 2025.

Registration and cancellation of My Number card for health insurance card use

Registering your My Number card as a health insurance card

 To use your My Number card as a health insurance card, you will need to register for the first time on My Number Portal (see below).
You can also register for the first time at your city hall or branch office, so if you would like to register, please check your PIN and bring your My Number card.

Canceling registration of My Number card as a health insurance card

 If you are already registered and would like to cancel your registration, please fill out the application form below and submit it to your city hall or branch office.

* If the applicant is different from the insured person, a separate letter of attorney (any format) is required, so please submit it together.

Benefits of My Number Insurance Card

・If medical expenses become high, there is no need to apply for or present a certificate of limit application.
- Medical institutions can also check past medication information and health checkup results, allowing you to receive better medical care.
