Comprehensive medical examination fee subsidy (under 40 years old)

In order to maintain and improve health, if members of Inagi City National Health Insurance voluntarily undergo comprehensive medical examinations at medical institutions, etc., part of the costs incurred for the examination will be subsidized.

Those who meet all of the following conditions

• As of the day of the examination, the patient is enrolled in the Inagi City National Health Insurance.
• 20 years old or older and under 40 years old (note) as of the date of examination.
• We pay national health insurance tax in full.

Note: Examinations covered by insurance are not covered by the subsidy.

Amount equivalent to 1/2 of the examination fee paid by the examinee
Note: If the amount equivalent to half of the examination fee exceeds 20,000 yen, the upper limit is 20,000 yen.

Once a year

Please bring the following items to the Health Insurance and Pension Section or Hirao/Wakabadai branch office to apply.

• insurance card
• Receipt
• Something that shows the inspection items (description, etc.)
• Household head's seal (stamps such as Shachihata are not acceptable)
• Something that shows the transfer destination (passbook, etc.)

From 8:30 a.m. to 5:00 p.m. of open agency day Remarks: From 8:30 a.m. to noon, 1:00 p.m. to 5:00 p.m. on holiday open agency day Note: Only city hall is open agency on holiday.
