Identity verification documents/personal number (My Number) verification documents

In order to apply for National Health Insurance, with some exceptions, "personal identification documents" and "personal number (My Number) confirmation documents" are required.

Note: Personal number notification card is not accepted, passport, basic resident register card, disability certificate
