Eligible Individuals

Individuals aged 20 and over who have a significantly severe mental or physical disability and require constant special care in their daily lives.

Amount Granted

Monthly amount: 28,840 yen (revised from April 2024)
Payments for three months up to the previous month will be made around the 10th of May, August, November, and February.
(There are some exceptions, such as for those who have been newly certified or have lost their qualifications.)

Degree of Disability Subject to Eligibility

Those who fall under any of the following 1 to 4.

  1. Those who have two or more disabilities as defined in 1 to 7 below
  2. Those who have one of the disabilities specified in 1 to 7, and also have two other disabilities equivalent to the second level of the Basic Disability Pension, totaling three disabilities.
  3. Individuals who have one of the disabilities specified in 3 to 5 below, and whose evaluation of daily living activities is extremely severe due to the particular severity of that disability.
  4. Those who have one of the disabilities specified in 6 to 7 below, and whose condition is in absolute rest or has a mental disability, have an extremely severe evaluation of daily living abilities.

"Ordinance for the Enforcement of the Law Concerning the Payment of Special Child Allowance, Appendix 2"

  1. The following visual impairments
    1. Those with a visual acuity of 0.03 or less in each eye
    2. Vision in one eye is 0.04, and the vision in the other eye is below manual valve.
    3. As a result of measurements using the Goldman perimeter, the sum of the peripheral visual field angles for both eyes with the I/4 target is 80 degrees or less, and the central visual field angle for both eyes with the I/2 target is 28 degrees or less.
    4. Results of measurements using an automatic perimetry indicate that the number of visual points in open binocular vision is 70 or less, and the number of visual points in binocular central vision is 20 or less.
  2. Hearing level of both ears is 100 decibels or more
  3. Those who have significant disabilities in the functions of both upper limbs, those who are missing all fingers of both upper limbs, or those who have significant disabilities in the functions of all fingers of both upper limbs.
  4. Those with significant disabilities in the function of both lower limbs or those who are missing both lower limbs above the ankle.
  5. Those who have a disability that prevents them from sitting up to the extent of being unable to maintain trunk function or standing up.
  6. In addition to the items listed above, conditions that require long-term rest or have functional disabilities of the body that are recognized to be at least equivalent to the above items, to the extent that they render daily life impossible.
  7. A mental disability that is recognized to be at least at the same level as the previous items

Payment Restrictions

Those who fall under any of the following categories cannot apply or receive benefits. For more details, please contact the Disability Welfare Division.

  1. Those whose income exceeds the income limit (Note)
  2. Individuals residing in the facility
  3. Persons hospitalized in a hospital or clinic for more than 3 months

Note: Whether or not the income limit is exceeded will be determined based on the amount after deductions from the income of the individual or their spouse/dependents. For more details, please refer to the income limit table and deduction amount table below.

If you meet the following conditions, you can receive an increase in the limit amount.

  • Income of Eligible Recipients
    • If there are elderly dependent spouses or elderly dependents among the dependents, the deduction is 100,000 yen per person.
    • If there are specific dependents or dependents eligible for deductions (limited to those aged 16 to under 19), the amount is 250,000 yen per person.
  • Income of spouse and dependents (when there are two or more dependents)
    When there are elderly dependents, the amount is 60,000 yen per person (if there are no other dependents besides the elderly dependent, it is 60,000 yen for one elderly dependent excluding one elderly dependent among them).

Deductions will be made from the income of the individual or their spouse/dependents for the above items, and income assessment will be conducted.

  1. Application for Special Disability Allowance Certification
  2. Special Disability Allowance Income Status Report
  3. Special Disability Allowance Certification (Designated Format)
  4. Application for Direct Debit of Special Disability Allowance (Please specify the account in the name of the applicant)
  • Note: Those who receive the Severe Physical and Mental Disability Allowance do not need a medical certificate.
  • Note: There are fields to enter your personal number (My Number) in items 1 and 2.

  1. Application for Dependent Relatives Eligible for Deductions Aged 16 to Under 19
  2. Copy of the Pension Certificate for Eligible Recipients (in case of receiving Disability or Survivor's Pension)
  3. (Non) Taxation Certificate
    Note: The (Non) Taxation Certificate can be abbreviated by entering the personal number (My Number) in 1 and 2.
