Physical Disability Certificate
This is a certificate necessary for individuals with physical disabilities to receive various forms of support. It is issued when recognized as corresponding to the types and degrees of disabilities defined by the Physical Disability Welfare Act. The grades of the certificate range from Grade 1 (most severe) to Grade 6 (mild).
The types of disabilities include visual impairment, hearing impairment, balance function impairment, speech, language or chewing function impairment, physical disability, and internal function impairment.
In principle, there is no renewal; however, if changes such as improvement in the condition of the disability are anticipated, re-certification may be conducted after a certain period following the issuance of the certificate.

Application Method
Please bring the following items and apply at the Disability Welfare Division counter in principle.
- Application Form (Available at the Disability Welfare Division counter)
- Physical Disability Certificate and Opinion Statement (created by a designated physician as stipulated in Article 15 of the Disability Welfare Act within the last year)
- Photo (4cm tall x 3cm wide. No hat, upper body, taken within one year from the application date)
- My Number Card Related Documents
- Note: The application form is in the format of Tokyo Metropolis and is a carbon copy type, so it cannot be downloaded from the city website or other sources.
- Note: It may be possible to accept applications for services at the same time as applying for the handbook.
In that case, depending on the services applied for, documents that identify the applicant's account and a copy of the Health Insurance Card may be required. Additionally, there are eligibility requirements for the services, so please contact the Disability Welfare Division for details.

Only doctors designated under Article 15 of the Physical Disability Welfare Act can create Physical Disability Certificates and Opinion Statements. The format varies depending on the type of disability. You can download it from below.

Other Topics
- Based on the application, an examination will be conducted by Tokyo Metropolitan Government, and if a grade is determined, a handbook will be issued. There may be cases where a handbook is not issued, or where a grade different from that stated in the medical certificate or Opinion Statement is issued.
- It takes about 2 months until the notebook is issued.
- In the following cases, procedures will be required. For details about the procedures, please contact the Disability Welfare Division.
- When the situation of the disability changes or when adding new information
- When there are changes to your address or name
- When you lose or damage your notebook
- When the holder of the notebook passes away
- When you no longer qualify for the disability handbook
