Personal Record Notebook Support Book Inagi "Powar"

"Powar" is a tool that allows individuals with developmental challenges who require personalized support to manage medical information, obtain certificates, receive diagnoses, and track their physical conditions and welfare service usage all in one file.
By engaging with hospitals, health centers, various administrative and consultation agencies, and local clubs and organizations, it can reduce the burden of having to explain the same things repeatedly.
At each institution, individuals can document and file what they have learned and the considerations they have noticed with the help of the staff, allowing the next supporter to review the filed formats and materials, thereby facilitating the transfer of support across life stages and sharing necessary information at each stage.
