This is a necessary certificate for individuals with mental disabilities to receive various forms of support, issued by the governor of the prefecture (hereinafter referred to as "certificate" for the mental disability welfare certificate on this page).
It is a necessary certificate for those recognized as having a mental disability as defined by the laws concerning mental health and welfare for individuals with mental disabilities, to receive various support systems in daily and social life. It is issued in grades from 1 to 3 based on the state of mental illness (mental disability) or the state of functional disability. Additionally, those who already possess a certificate and are recognized as having a change in the degree of disability can apply for a grade change.

Application Method

Please bring the following items to the Disability Welfare Division counter or apply by mail.

  1. Application Form
  2. Medical certificate (limited to those that have passed more than 6 months from the initial diagnosis date related to mental disorders (as of the time of creating the medical certificate). The form can also be downloaded from the Tokyo Metropolitan Central Comprehensive Mental Health Welfare Center website.)
    Note: If you are receiving a disability pension based on a mental disorder, the medical certificate is not required if you submit a consent form along with one of the following documents.
    • Copy of Disability Pension Certificate
    • Copy of Disability Pension Decision Notification
    • Copy of Disability Pension Payment Notification
  3. Photo (4cm tall x 3cm wide. No hat, upper body, taken within one year from the application date) 1 piece
  4. My Number Card Related Documents
  5. Copy of your notebook (in case of renewal)
  6. Reply envelope (for mail applications only. Attach a 110 yen stamp)

About the Application Form

If you wish to receive the necessary documents for your application, you can apply using the form below. It will take about two weeks from your application until the documents arrive, so if you are in a hurry, please come directly to the Disability Welfare Division counter.

Notes on Updates

The validity period is 2 years.

You can apply for renewal starting three months before the expiration date.
We provide monthly renewal notifications through the Inagi City email distribution service.

Other Topics

  • Based on the application, an examination will be conducted in Tokyo, and if the grade is determined, the handbook will be issued. Depending on the examination results, the handbook may not be issued, or a different grade may be determined compared to before the renewal.
  • It takes about 3 months until the handbook is issued.
  • When applying for both the disability certificate and the self-support medical care (mental outpatient) at the same time, it may be possible to apply using only the diagnosis certificate for the disability certificate.
