About benefit individual consultation (information for companies)

When we hope for introduction of the following services, prior individual consultation is necessary for city.

(1) In-hospital assistance in home-visit nursing care (2) Life support such as home-visit nursing care services when there are family members living together (3) Rental of welfare equipment for mild cases (limited to cases where confirmation by the city is required)
(4) Use of short-term inpatient living care (short-term inpatient recuperative care) for more than half of the valid period of certification (5) Other items for which individual consultation is considered necessary Please check.

• In the case of content (1), (2), (3), and (5), before service introduction
• In the case of content (4), before it is expected to exceed half of the certification validity period
• If it is determined that the benefits are appropriate and you want to continue using the service after the applicable period ends, the date before the end of the applicable period.

Note 1: Please consider that it will take approximately two weeks to review the submitted documents after receiving them.
Note 2: Even if you are unable to submit the form before the deadline for some reason, please be sure to notify the Benefits Section.
Note 3: If the service is judged to be “inappropriate for benefits” after the introduction of the service, the service will not be covered by nursing care benefits.
