About the care plan review meeting (care plan inspection)

Care plan review meeting

In city, we carry out care plan review meeting as part of payment adequacy.

Implementation policy and purpose

This is done as part of the optimization of benefits, but rather than a teaching method, the "re-assessment support sheet" is used to conduct an assessment (problem analysis) together with the city, the chief nursing care support specialist, and the nursing care support specialist. By doing so, we encourage long-term care support specialists to "awareness", pursue together what is "care management that contributes to independence support", and promote its universalization to support the implementation of sound benefits. . We will also improve the quality of care management by long-term care support specialists.

Target

Long-term care support specialists, in-home long-term care support offices, small-scale multi-functional in-home long-term care offices, nursing small-scale multi-functional in-home long-term care offices belonging to the following offices located in the city

Implementation method

(1) The city will request business establishments to attend the review meeting. (approximately 2 months before the scheduled implementation)
(2) The nursing care support specialist at the business will submit advance materials such as a care plan (pre-submission form, basic information sheet, re-assessment support sheet, home service plan) to the city. (Approximately 1 month before the scheduled implementation)
(3) We will hold a care plan review meeting and inspect the care plan. (Usually held on the afternoon of the third Tuesday of each month)

Participants of the review meeting

  • Long-term care support specialist who submitted the care plan, etc. and witnesses
  • Chief long-term care support specialist (community comprehensive support center, city home long-term care support office, etc.)
  • Elderly Welfare Division Nursing Care Insurance Section
