Requesting a copy of your residence certificate
Application location

- Convenience store [For those who have a My Number card (equipped with an electronic certificate for user verification)]
Who can apply
- The person or a person from the same household as the person
- An agent with a power of attorney
Note: The power of attorney must be filled out by the delegator. (Download the application form and go to the "About Power of Attorney " page)
What you need to make a claim
- Identity verification documents for applicants who come to the counter (For identity verification documents, please see the "Identity Verification for Various Notifications/Requests" page.)
- Documents confirming the authority of a legal representative such as an adult guardian (we may ask you to confirm the contents of the document)

Points to note when requesting a copy of your resident record
- For requests by Japanese residents, unless there is a special request, the domicile, principal person, relationship, name of head of household, resident record code, and individual number (My Number) are generally omitted.
- Regarding requests by foreign residents, unless there is a special request, information such as relationship and name of head of household, nationality/region, status of residence, common name history, resident record code, and individual number (My Number) will be omitted in principle.
- In the case of a third-party request, explanatory materials (related materials) are required according to the reason for the request.
- If a proxy requests a copy of the residence certificate or a certificate of details containing the residence certificate code or personal number (My Number), the document will be sent to the proxy by mail. Please note that these documents cannot be issued on the spot.

- Please fill in the necessary information on the document for which you wish to receive certification.
- If you make a mistake, please rewrite it on a new sheet of paper without making any corrections.
