- Your personal seal registration card or My Number card
When a representative visits the office
- Your personal seal registration certificate (card)
Note: If you lose your seal registration card, you will not be able to request a seal registration certificate even if you have your My Number Card. In that case, you will need to re-register your seal.

Note: Only City Hall is open on holidays.

- If an application is made by a representative, the registrant's address, name, and date of birth must be correctly recorded on the application form.
