• Your personal seal registration card or My Number card

When a representative visits the office

• Your personal seal registration certificate (card)

Note: If you lose your seal registration card, you will not be able to request a seal registration certificate even if you have your My Number Card. In that case, you will need to re-register your seal.

Available date and time

From 8:30 a.m. to 5:00 p.m. of open agency day Remarks: From 8:30 a.m. to 11:45 a.m., 1:00 p.m. to 5:00 p.m.
Note: Only City Hall is open on holidays.

• If the application is made by a proxy, it is a condition that the registrant's address, name, and date of birth can be entered correctly on the application form.
