Notification of Change of Head of Household/Notification of Separation/Merger/Change of Household Composition

Notification is required when there is a change in the head of the household, separation, merger, or change in composition of household members.
Notification period
Within 14 days from the date of change
Note As a general rule, changes will be made on the date of notification.

- City Hall 1st floor, Citizens Affairs Division
- Hirao Branch
- Wakabadai branch office (i-Plaza 1st floor)
Note: Changes to households that include foreign residents cannot be processed at branch offices. Please come to the Citizens Section on the 1st floor of City Hall.
Person who filed the notice
- Changed person or head of household
- An agent with a power of attorney
Please note that all power of attorney documents must be personally signed by the delegator.

What you need for notification
Identity verification documents of the notifier
Note Please check the page of "Identity verification at the time of various notifications and requests" for identity verification documents of the notifier.
Required only if applicable
- For foreign residents whose household composition has changed and who cannot demonstrate their relationship with the head of the household, a certificate and translation showing this.
- National health insurance card
- Elderly elderly insurance card
- Nursing care insurance card
- Infant medical certificate etc.
Other necessary items and procedures will vary from person to person. If you have any questions, please contact the relevant departments as soon as possible.

8:30am to 5pm on business days. Note: 8:30am to 11:45am and 1pm to 5pm on holiday days.
Note: Only City Hall is open on holidays.
