The notification card is a paper card that informs you of your 12-digit personal number (My Number).

The notification card is used as a document to prove your My Number when you are asked to present your My Number for procedures related to social security and taxes.

  • Note: The documents that can be used to prove your My Number are limited to cases where the information on the notification card (address, name, etc.) matches the Certificate of Residence.
  • Note: This card includes your name, address, date of birth, and gender, but since it does not contain a photo, it cannot be used as a form of identification on its own.

The design of the card is as follows.

The notification card system has been abolished (Changes to the notification card details and reissuance are no longer possible)

Due to the amendment of the law, the notification card system has been abolished, and as of May 25, 2020, changes to the information on the notification card (such as address and name) and reissuance are no longer possible.

Even if the notification card system is abolished, if the information on the notification card (address, name, etc.) matches the Certificate of Residence, it can continue to be used as a document to prove the My Number.

In addition, you can still apply for a My Number Card even after the notification card is abolished.

Method to Confirm My Number After the Abolition of Notification Cards

  1. My Number (Personal Number) Card
    For information on how to obtain it, please refer to the page on "My Number Card (Personal Number Card)."
  2. A copy of the Certificate of Residence or a Certificate of Residence Details that includes the My Number
    For information on how to obtain these, please refer to the "Certificate of Residence" page.
  3. Notification card (limited to cases where the address and name on the notification card match those on the Certificate of Residence)

Notification method for those who are receiving their My Number for the first time (such as those who have lived abroad or were born)

For those who are receiving their My Number for the first time, we will inform you of your My Number through the "Notification of Individual Number".
Please refer to the "Notification of Individual Number" for details.

Note: This notification is for notifying the My Number, and cannot be used as a document for verifying the My Number or for identity verification.
Additionally, reissuance is not possible.
If you need a document to prove your My Number, please obtain the My Number Card or a "Certificate of Residence" or "Certificate of Residence Details" that includes your My Number.
