When your Individual Number Card for those emigrating overseas is lost, burned, or damaged

When you lose your My Number Card

If you lose your My Number Card, please follow the steps below.

  1. First of all, in order to prevent abuse of the card, please contact the call center below and take the procedure for temporary suspension.
  2. After the suspension, please submit a lost property report to the nearest police box or police station. Please note that the lost property report acceptance number will be required for the procedure to reissue your My Number card in the future, so please be careful not to lose it. (If you lose your item overseas and submit a lost property report overseas, you will need to attach a translation.)
  3. If you wish to cancel your lost My Number Card and have it reissued, please complete the procedure at a Japanese diplomatic mission overseas, the city or town where you are temporarily staying, or your place of permanent domicile (a fee will be charged).
  4. If you find your lost card, the suspension will be lifted, so please go through the procedure at the Japanese diplomatic mission or your city, town or village where you are registered.

Those who have abolished my number card and wish to obtain a new card (reissuance of my number card)

If your My Number card is lost, burned, or damaged, you can apply for cancellation and re-issuance at the Japanese Embassy or Consulate-General overseas, the city or ward where you are temporarily staying, or your permanent domicile. If your lost My Number Card is found, you can use it again by going through the procedure to unsuspend it.

Place of application (registered domicile is Inagi City)

  • Diplomatic missions overseas (The process will take some time as you will need to set up your PIN in Inagi City.)
  • Inagi City Hall Citizens' Affairs Division, Hirao Branch Office, Wakabadai Branch Office (You can complete the procedure on the spot)

Apply in person

(1) Found My Number Card Application for overseas embassy: PIN must be written on the application form Application for Inagi City: PIN must be verified at the counter

Application for legal representative (if the person in question is under 15 years old or an adult ward)

(1) Your Individual Number Card
Overseas diplomatic mission application: PIN code must be entered on the application form
Inagi City application: PIN verification is required at the counter. (2) One government-issued photo ID of the legal representative (passport, My Number card, etc.)
(3) [If the person is an adult ward] Certificate of registered matters

Those who have issued digital signature certificates

Even if you unsuspend your My Number card, the electronic signature certificate will not be automatically cancelled. After unsuspending your My Number card, you will need to go through the process of revoking and reissuing it. In that case, you will need to enter the PIN number (two types: 4 digits and the electronic signature certificate) for the My Number card that was found.
