Supportive Medical Care (Mental Outpatient)
This is a system that subsidizes medical expenses for continuous treatment due to mental illness through outpatient visits. The patient's share is generally 10%. However, depending on the individual's income, the income of the insured household, and the condition, a monthly maximum limit on the patient's share may be set, or the entire patient share may be exempted.
Home visits, day care, home nursing, treatment for epilepsy, and medication costs related to mental outpatient care are also covered. Medical expenses related to hospitalization are not included.
Application Method
Please bring the following items to the Disability Welfare Division counter or apply by mail.
- Application Form
- Medical certificate (can also be downloaded from the Tokyo Metropolitan Central Comprehensive Mental Health Welfare Center website.)
- Note: Must be created by a physician within 3 months.
- Note: Since it is submitted once every two years, it may not be necessary to update it.
- Any of the following insurance information verification documents (not required for those under the Inagi City National Health Insurance or Late-Stage Elderly Medical Care system.)
- Copy of Health Insurance Card
- Copy of the "Eligibility Confirmation Document" issued by the health insurer
- Copy of the "Qualification Information Screen" that can be checked from the My Portal
- Note: If the person receiving the examination is a member of the National Health Insurance Association (such as the National Civil Engineering and Construction National Health Insurance Association, Tokyo Barber National Health Insurance Association, etc.), insurance information confirmation documents for all household members who are enrolled in the same insurance as the recipient are required.
- Household Income Statement and Consent Form
- Certificate of Eligibility for Medical Support for Independence (Mental Health Outpatient) (for Renewal)
- Taxation (Non-Taxation) Certificate (Required for those moving in, if necessary. Please contact us in advance.)
- Documents that confirm the name and location of medical institutions and pharmacies (such as medical examination tickets and medicine bags)
- Documents related to My Number
- Reply envelope (for mail applications only. Attach a 110 yen stamp)
About the Application Form
If you wish to receive the necessary documents for your application, you can apply using the form below. It will take about two weeks from your application until the documents arrive, so if you are in a hurry, please come directly to the Disability Welfare Division counter.
Notes on Updates
The validity period is generally one year.
Renewal applications can be processed starting three months before the expiration of the validity period.
Renewal notifications are sent out every month via the Inagi City email distribution service.
Other Topics
- Based on the application, an examination will be conducted in Tokyo, and if approved, a certificate for self-supporting medical care (mental health outpatient) will be issued.
- It takes about 3 months to issue the recipient certificate.
- Can only be used at designated medical institutions listed on the recipient certificate.
- If there are no changes in the medical condition and treatment policy, the submission of the Self-Support Medical Certificate will be required once every two years. However, if the application is made after the validity period has expired, it will be considered a reapplication, and the submission of the certificate and other documents will be necessary.
- When applying for both Independence Support Medical Care (Mental Outpatient) and the Mental Disability Welfare Handbook at the same time, it may be possible to apply using only the diagnosis certificate for the handbook.
- Updates, changes to medical institutions, changes to health insurance, changes of address, changes of name, and reissuance due to loss or damage require procedures.
- If you have a Mental Disability Welfare Handbook issued based on a diagnosis certificate, you may be able to submit a copy of the handbook instead of the diagnosis certificate when applying. For more details, please contact us.
