Inagi City Dementia Early Support Promotion Project

This is a home support team for dementia, composed of dementia specialists, healthcare and welfare professionals (nurses, Public Health Nurses, occupational therapists), and others.

What can you do for me?

We visit the homes of individuals with dementia or those suspected of having dementia to provide consultations regarding their concerns and issues related to dementia. Based on this, we will focus on providing initial support related to dementia, including coordinating visits to appropriate hospitals and various services to alleviate the caregiving burden on families. (Generally for about 6 months)

Eligible Persons

Individuals aged 40 and over who live at home and have difficulty responding due to symptoms of dementia or are suspected of having dementia.

For example, if you are having trouble with someone like the one below, please consult us.

• Treatment for dementia has been interrupted
• We recommend the use of nursing care services, but progress is slow.
• Dementia is suspected, but the individual is refusing to visit a medical institution.
• The symptoms of dementia are severe, and I am struggling to cope.

Those with symptoms as described above are eligible.

You can contact us by phone or visit us in person. Staff from the Community General Support Center can also visit your home.
