When a person enrolled in the National Health Insurance Program of Inagi City passes away, a funeral expense of 50,000 yen will be provided to the person who conducted the funeral upon application from the funeral executor (chief mourner).
Please note that this payment will not be made if more than two years have passed since the day of the funeral.

Note: In the case of death due to the actions of a third party (such as a traffic accident), funeral services may not be provided if the third party bears the funeral costs as compensation.

Required Items for Application

  • Health Insurance Card or Qualification Confirmation Document of the Deceased
  • Documents that indicate a funeral was held (funeral invitation or a receipt/invoice for the funeral with the name of the chief mourner)
  • Information on the transfer destination

For mail applications, please attach the completed Application Form and the documents mentioned above.
