Health Checkup Subsidy (For those aged 40 and over)

Since the fiscal year 2014, even those over 40 years old can receive assistance if they undergo a comprehensive medical checkup at the Inagi City Hospital Health Checkup Center.

Eligibility

Those who receive a comprehensive medical checkup at the Inagi City Hospital Health Checkup Center and meet all the following requirements on the day of the checkup

• Insured under the National Health Insurance of Inagi City
• Individuals who are eligible for specific health checkups and have not yet undergone the relevant specific health checkup
• Has fully paid the National Health Insurance Tax that has reached the payment deadline

Note: If the examination is covered by insurance, it is not eligible for assistance.

Subsidy Amount

Half of the examination fee (up to 20,000 yen)

Application Method

Please make a direct reservation at the Inagi City Hospital Health Checkup Center. When you visit, please bring the following items and apply for the examination fee subsidy at the counter of the Inagi City Hospital Health Checkup Center. You will be able to receive the examination at the amount after the subsidy has been deducted in advance.

• Application Form (After your reservation, it will be sent along with the information for the health checkup from the Inagi City Hospital Health Check Center.)
• Seal
• One of the valid Health Insurance Card, My Number Health Insurance Card, or Qualification Confirmation Document
• Specific Health Checkup Voucher
• Inagi City Hospital Consultation Ticket (for those who have one)
