Health Checkup Subsidy (for those under 40 years old)

To promote and maintain health, if a subscriber of the Inagi City National Health Insurance voluntarily undergoes a health checkup at a medical institution, a portion of the costs incurred for that examination will be subsidized.

Eligibility

Those who meet all of the following conditions

• As of the date of examination, I am enrolled in the National Health Insurance of Inagi City.
• As of the date of examination, the individual is between 20 and 40 years old (note).
• The National Health Insurance Tax has been fully paid.

Comprehensive Medical Checkup
Note: If the examination is covered by insurance, it is not eligible for assistance.

Subsidy Amount

An amount equivalent to half of the examination fee paid by the examinee
Note: If the amount equivalent to half of the examination fee exceeds 20,000 yen, the limit is 20,000 yen.

Once a year

Application Method

Please bring the following items to the Insurance and Pension Division or Hirao/Wakabadai Branch Office and apply.

• One of the following: a valid Health Insurance Card, My Number Card, qualification confirmation document, or qualification information notice.
• Receipt
• Items that indicate the inspection details (such as statements)
• Documents that indicate the transfer destination (such as a bankbook)

Within 2 years from the date of examination

Open hours are from 8:30 AM to 5:00 PM
Note: On holidays, open hours are from 8:30 AM to noon and from 1:00 PM to 5:00 PM
Caution: Holiday openings are only at City Hall.
