Note: Only City Hall is open on holidays. Alternatively, you can request by mail or have it delivered at a convenience store.

Regarding request for mailing of taxation certificate/tax exemption certificate

If you wish to request a taxation (tax exemption) certificate by mail, please enclose the following four items and send it to the Taxation Division.

1 Mail request application form

Please download the application form from the following application form download page. Please follow the example of the mail-in application form you received. Letter paper ( ) You may also write it in the form below.

2 Fixed amount postal money order ( ) (Number of requests)

This is required as a handling fee, so please purchase it at the post office.
(1) If you send the certificate by mail, it will cost 400 yen per certificate. Please enclose an envelope with the same amount as the number of certificates requested, plus the correct change. Money ( ) Please refrain from doing so .
(2) The validity period is six months from the date of issue. For administrative reasons, if the remaining validity period is more than 10 days , Fixed amount postal money order ( ) Please send the following.
(3) Fixed amount postal money order ( ) Please send it blank on both sides .

3.Reply envelope (with a return stamp and the applicant's current address and address written on it)

4 Copy of identity verification document (in the case of a representative, the representative's identity verification document)

5 Power of attorney (if an agent applies)

A personal signature of the person certifying is required.
Note: If you are not an Inagi citizen at the time the application is received, a power of attorney is required even if you are a relative who lives with you .
Example: If you need certificates for both husband and wife, you will need two application forms and one power of attorney (from husband to wife, from wife to husband, etc.).
