Delivery after starting use

Documents required after starting use

If you start using the facility for the following reasons, please submit the prescribed documents to the Child Care Support Division.

Please note that if you do not submit your application after 3 months, you will be asked to leave and reapply.

Employment certificate (thing of certification day after work)

Please note that if you do not submit your application within one month, you will be asked to leave and reapply.

Returning to work after maternity/childcare leave

certificate of absence

If it turns out that you have not returned to work on the certificate of employment submitted during the month you started using the service, your job offer will be canceled and you will be discharged. (Return to work in the employment situation at the time of application is a condition.)
A certificate issued after returning to work is required.

Various procedures while in kindergarten

In the following cases, please be sure to promptly submit the necessary documents to the Child Care Support Division.

[If you decide to work]
Submit (1) by the month before the month you plan to start working, and submit (2) after you start working.
[Those who have applied for and started using a nursery school under the conditions of a job offer]
Submit (2) after starting work. Please note that if you cannot start working during the month you start using the facility, you will be discharged.

resignation

(1) Notification of change of family situation

Submit the required documents by the month before the month you plan to leave your job.
If you cannot start working within 3 months after leaving your job, you will be discharged.

childbirth

(1) Notification of change of family status (2) A copy of the mother and child handbook (the page showing the names of the parents and the expected date of delivery)

Please submit the required documents two months before the expected month of childbirth or the month before the expected start of maternity leave. The certification period is until the month after the month of birth.
[When returning to work after childbirth]
Submit (1) by the month before the month you plan to return to work, and submit a certificate of absence after returning to work.
[When taking childcare leave after maternity leave]
The procedure is the same as for taking childcare leave below.

Taking maternity/childcare leave

(1) Notice of change of family situation (2) Certificate of absence from work

[When taking maternity/childcare leave]
Submit (1) by the month before the month you plan to start maternity/childcare leave, and submit (2) after taking maternity/childcare leave.
Note) In the case of taking childcare leave, certification of the older child during childcare leave will be short-time certification. (If a child already enrolled in a nursery school takes childcare leave, the child will be discharged.)
[When returning to work]
Submit (1) by the month before the month you plan to return to work, and submit (2) after returning to work. ((2) is valid only on the certification date after returning to work)
[Those who have applied to use a nursery school to return to work and have been accepted]
Please submit only (2) after returning to work.

Change of name/family structure (marriage, divorce, etc.)

(1) Notification of change of family situation (2) Certificates such as family register

[In case of marriage or divorce]
Please submit (1) and (2) within the month after the marriage or divorce is established. (In the case of marriage, submit only (1))

wish to transfer

(1) Transfer application form

The same attachments as new applications are required.
(Can be omitted if submitted for the same year's application)

wish to rest

(1) Nursery school holiday notification (2) Medical certificate, certificate, etc. specifying the holiday period

The maximum period of leave that can be taken is two months. If you are absent for more than 2 months, you will be asked to leave and reapply.
Childcare fees can be exempted only if the child cannot attend kindergarten for physical reasons and is absent from the school for a period of one to two months.
Note: Please contact us if you are using childcare moms.

wish to leave

(1) Notification of leaving school

Please submit by the 15th of the desired month.
If you do not submit it, you will be deemed to be in the nursery school and will be required to pay the childcare fee for the following month. (If you use childcare mama, please contact the childcare mama.)

moving out of the city

(1) Nursery school leaving notice

Please submit by the 15th of the month you move out.
If you would like to continue attending the current facility after moving out, please contact us.
(If you are a childcare mom, please contact the Child Care Support Division.)

Student (at school)

(1) Notice of change of family situation (2) Certificate of enrollment (3) Schedule

Care/Nursing

(1) Notice of change of family situation (2) Medical certificate of care recipient/nurse or copy of disability certificate, etc. (3) Schedule

Illness/Disability

(1) Notification of change of family situation (2) Copy of medical certificate or notebook

Note: The medical certificate must state the length of treatment and the difficulty of home care.

About the current situation survey

Inagi City conducts a survey on the current status of education and childcare benefits certification for children (once a year) to confirm the need for childcare.
Please note that if the application is not submitted by the deadline, the need for childcare cannot be confirmed and the child will be discharged.
