Daily Living Equipment Purchase Cost

We provide financial assistance for the purchase of welfare equipment necessary to improve the convenience of daily life for individuals with disabilities.

Eligible Individuals

In principle, individuals with a Physical Disability Certificate, Love Certificate, or Mental Disability Health and Welfare Certificate who are severely disabled and live at home.

  • Note: The eligible participants vary depending on the category.
  • Note: Those eligible for Long-Term Care Insurance should use the Long-Term Care Insurance system for eligible services.
  • Note: Among patients with intractable diseases, those deemed necessary may be eligible for benefits.

Cost

As a general rule, 10% of the standard amount for the items purchased (the standard amount varies by item) will be the user's burden, and there is a certain upper limit on the burden based on the household's income. Additionally, any amount exceeding the standard amount will be fully borne by the individual.

Procedure

You cannot receive assistance after purchasing daily living equipment. Please be sure to consult with us before making a purchase.

Before visiting the office, please consult using the form above or by phone.
(This is to avoid the inconvenience of having to visit again if the person in charge is not present when you come. Additionally, it may be possible to reduce the number of visits by sending necessary documents by mail.)

Payment Method

Costs will be provided either by "reimbursement payment" or "proxy receipt".

  • Reimbursement payment... A method where the full amount is paid to the contractor upfront, and the subsidy amount is received from the city later.
  • Proxy receipt... Only the self-burden amount is paid to the contractor, and the remaining amount (subsidy amount) is paid by the city to the contractor.

Note: The proxy receipt method may take longer than the reimbursement payment method because a contract must be concluded between the city and the contractor.

Housing Equipment Improvement Costs

We provide subsidies for renovation costs such as entrances to facilitate daily living for individuals with severe physical disabilities living at home (school-age and older). However, if benefits for housing renovations can be received from Long-Term Care Insurance, the Long-Term Care Insurance system will take precedence.

Costs, Procedures, and Payment Methods

Similar to daily living tools. Please be sure to consult before renovations.
