Documents related to My Number required when applying for allowance (by submission method)

When the applicant himself/herself completes the procedure
(1) Documents that can confirm your Individual Number
- Individual number card
- notification card
- Certificate of residence (my number listed), etc.
Note: Please be sure to bring the original My Number confirmation documents of the applicant . (Not copyable)
Note: It is not necessary to present the original My Number confirmation documents for those other than the applicant (spouse, child, dependent person, etc.).
(2) Identity verification documents of the applicant
[Things that can be confirmed with one point]
- Individual number card
- License
- passport etc.
Note: One official document with a photograph of the face is subject to verification.
[Things that can be confirmed by two points]
- Health insurance card
- pension book
- Child Rearing Allowance Certificate, etc.
Note: Official documents without a photograph are subject to two-point verification.
When a representative (spouse, etc.) completes the procedures
(1) Documents that can confirm your Individual Number
- Individual number card
- notification card

When applying by mail
(1) A copy of a document that can confirm the applicant's My Number
- Copy of Individual Number Card

- A copy of your resident card (indicating your My Number), etc.
Note: Be sure to mail a copy . Mailed originals will not be accepted.
