Medical expenses subsidy system for compulsory school children

Compulsory education children who live in Inagi City and are enrolled in health insurance (first grade of elementary school to third grade of junior high school, up until the first March 31 after turning 15)
There are no income restrictions.
Note: For households receiving welfare assistance, people living in institutions specified by the rules, and people entrusted to foster parents, priority will be given to the respective medical expense subsidy systems.
In addition, those who are receiving the Single Parent Medical Expense Subsidy Scheme or the Mentally and Physically Disabled Persons Medical Expense Subsidy Scheme without any out-of-pocket expenses (non-taxable households) will be given priority for the respective medical expense subsidy schemes.
Note : The applicant (guardian) must be the parent or guardian of a child attending compulsory education who is the primary breadwinner (the person with the higher income).

Scope of grant

Of the 30% of the self-pay portion of medical expenses covered by health insurance, subsidies are provided as follows.
Outpatient visit: Subsidize the portion excluding 200 yen per visit (upper limit of 200 yen for self-pay)
Hospitalization / Dispensing: Full subsidy (no self-pay)
Note: Expenses that are not covered by health insurance, such as health checkups, vaccinations, orthodontics, hospital meal charges, extra bed charges, additional charges for initial visits to non-referred patients at Inagi City Hospital, etc., are not covered by the subsidy.
In addition, if there are high-cost medical expenses or supplementary benefits paid by medical insurers (health insurance associations, etc.), subsidies will be provided for the amount after deducting the high-cost medical expenses and supplementary benefits from the copayment for medical treatment covered by insurance.

Grant start date

The start date of grant eligibility will be the first day of the month in which the application is made.
Note: In the case of moving in, etc., if the application is made by the same day of the following month, the start date of the subsidy qualification will be the day of moving in, etc., even if the month changes.

Application method

Please bring the documents listed below to the Childcare Support Section on the second floor of City Hall or to the Hirao/Wakabadai Branch Office to apply.
1 Medical certificate application form (available at the counter)
2. Health insurance card showing the child's name and date of birth
3. Documents showing the branch name and account number of the financial institution (in the applicant's or spouse's name, for a regular savings account)
4. My Number Verification DocumentsNote: You will need to write your My Number on the application form when applying.

Procedure for continuation

The fiscal year for the medical expenses subsidy system for compulsory school children changes every October.
We will continue to review every year. After screening, we will send a letter to those who need to confirm documents after August every year, so please follow the instructions.
For those who can continue to receive subsidies, we will mail a new medical certificate from October 1st.

There is a description on the back of the medical certificate to return it to the window of the Child Care Support Division, but please dispose of it yourself.

How to subsidize medical expenses

Subsidies are provided by presenting a compulsory education child medical certificate and a health insurance card together at the medical institution window at the time of medical examination.

If you have a medical examination outside of Tokyo or at a medical institution that does not handle medical treatment under this system

Please follow the steps in the link below to apply.

Cash benefits (subsidy for medical expenses such as out-of-town consultations, glasses for low vision, assistive devices, full payment, etc.)

Note : Applications must be submitted within 5 years from the day after the payment of medical expenses and within the validity period of the medical certificate.
Note : If you have other special medical expenses, you may need additional documents, so please inquire at the Child Care Support Division.
Note : It takes about two months from application to payment (account transfer).

When receiving benefits based on the Independent Administrative Institution Japan Sport Council Act

If your child becomes ill or injured at school, etc., benefits may be provided based on the Japan Sports Promotion Center Act. If you are instructed by your school, etc., please go to a hospital, etc. without using your medical certificate. (If you have not received the payment, please apply at the city office.)

Injury caused by the actions of others (third-party actions)

If you were injured due to the actions of another person and used your medical certificate together with your health insurance card, please contact the child care support section of the city hall.
Inagi City will send a document to the perpetrator to claim the costs.

If there are any changes after applying, please bring your medical certificate for compulsory school children and submit the following notifications.

Lost or damaged medical certificate for compulsory school children

Submit an application for reissuance of a medical certificate .
At a later date, we will mail a new medical certificate for compulsory school children.
(If you lose it, you do not need a medical certificate for compulsory school children)

Moved in Inagi City

Please submit a notification of change in medical certificate application items .
At a later date, we will mail a new medical certificate for compulsory school children.
When you receive your new compulsory education medical care certificate, please return the old one.

Moved out of Inagi City

Please submit a notification of change in medical certificate application items .
After moving out, please return the medical certificate for compulsory school children.
You will need to make a new application at the municipality where you are moving to.
Note: The Medical Expense Subsidy System for Children Enrolled in Compulsory Education is an independent project run by municipalities. Please check the system and documents required for application at your new destination.

If the recipient no longer takes care of the child due to divorce, etc., please submit a notification of change in the application for a medical certificate .
