Please fill out the application form and mail it to the address below or bring it to City Hall or Regional Comprehensive Support Center.
Application forms will be distributed at City Hall Elderly Welfare Division, Regional Comprehensive Support Centers (Hirao, Yanokuchi, Elegant Momura, Koyodai), and Hirao/Wakabadai business trips.
