QuestionPlease tell me how to request a certificate related to the family register by mail.

If you wish to request issuance by mail, please enclose the following four documents with your request.
- Mail Request Form (Please download from the "Forms for Family Register Certificate" page)
- Fees (Please pay with a fixed postal money order) Note: For the amount, please contact us.
- Return envelope (Please write the address of the registered residence and the name of the requester, and affix a stamp)
- Identification documents, etc. (Documents that can confirm the requester and the registered residence, such as copies of driver's license, Health Insurance Card, etc.)
