Notification regarding children living together

Pursuant to Article 30, Paragraph 1 of the Child Welfare Act, those subject to the law must report the matter to the prefectural governor via the mayor of the city or town.

Target audience

A child other than a child within the fourth degree of kinship is separated from the person with parental authority or guardian of a minor and is allowed to live with the child in one's own household (including single-person households) for more than three months (one month for infants). A person who has lived with the child willingly, or a person who has lived with the child continuously for more than 2 months (20 days or more for infants)

What you need for notification

• The above notification form
• Copy of resident record
• Identification of the person making the notification (driver's license, My Number card, Basic Resident Registration card with photo, etc.)
