Notification of children living together

Based on Article 30, Paragraph 1 of the Child Welfare Law, those affected must notify the prefectural governor via the mayor of the municipality.
Target audience
A child other than a child within the fourth degree of kinship is separated from a person who exercises parental authority or a guardian of a minor and is made to live in one's own home (including a single-person household) for more than three months (one month for infants). Those who intentionally let them live together or those who have continuously lived together for 2 months or more (20 days or more for infants)

Items required for notification
- the above notification form
- Copy of resident card
- Documents that can be used to verify the identity of the applicant (driver's license, My Number card, basic resident registration card with photo, etc.)
