Request for Re-registration of Evacuation Support Needs Registration Card
Starting from March of the 7th year of Reiwa, it will be necessary to re-register due to a review of the items in the evacuation support needs registry. We are sending a purple registration form to those currently registered, so please fill out the required information and return it using the enclosed reply envelope.
Note: If you do not return it by the end of June in the 7th year of Reiwa, you will be removed from the list of individuals requiring evacuation assistance.
Eligible Individuals
As of February 19, 2025 (Wednesday), the list of registered individuals requiring assistance in evacuation actions
Deadline
From March 1, 2025 (Saturday) to March 19, 2025 (Wednesday)
Note: However, applications will continue to be accepted sequentially after Thursday, March 20.

Purple Please fill out the evacuation support needs registration form and information provision consent form, and send it by mail using the enclosed reply envelope.

Information provided to local organizations such as Neighborhood Associations and Commissioned Welfare Volunteers will increase.
Before Change (Until the end of February, 2025)
The list of individuals requiring evacuation assistance will be shared only with local organizations such as each Neighborhood Association and Commissioned Welfare Volunteer, limited to the information of "names of individuals requiring evacuation assistance," "age," "address," "phone number," and "physical condition" for the respective area, to be utilized for disaster prevention and during emergencies.
After the change (from March 2025)
The list of individuals requiring evacuation assistance will be shared with local organizations such as each Neighborhood Association and Commissioned Welfare Volunteer, limited to the information you provide in the purple "Notification Form" which includes all information (the names of individuals requiring evacuation assistance, their ages, addresses, phone numbers, physical conditions, as well as the Physical Disability Certificate, Love Certificate, Mental Disability Health and Welfare Certificate, level of care, emergency contact, primary medical institution, and additional documents), for use in disaster prevention and during emergencies.
