The Late-Stage Elderly Medical Care Health Insurance Card can be used until the expiration date.
Starting from December 2, the transition to a system that primarily uses the My Number Card as a Health Insurance Card will take place, and the issuance of the Health Insurance Card will be discontinued. The Health Insurance Card you currently have can still be used until its expiration date, as long as there are no changes to your eligibility information.

Notice about Qualification Confirmation Documents and Qualification Information
Qualification Confirmation Document
If you acquire a new qualification or if your qualification information changes between December 2, 2024, and July 31, 2025, a qualification confirmation document will be issued without the need for an application, regardless of whether you have a My Number Card or not.
After that, for those who do not have a My Number Card, a qualification confirmation document that can be used from August 1, 2025, is scheduled to be issued around mid-July 2025.
(Note: The My Number Card refers to the My Number Card that has registered for Health Insurance Card use.)
Even if you do not have a My Health Insurance Card, you can receive insured medical treatment by presenting a qualification confirmation document at medical institutions, just like with the previous Health Insurance Card.

Those who are enrolled in Late-Stage Elderly Medical Care Insurance and have a My Health Insurance Card will receive a notification of eligibility information around mid-July of Reiwa 7.

To use the My Number Card as a Health Insurance Card, initial registration from the My Portal (see below) is required.
You can also complete the initial registration at the City Hall or Branch Office, so if you wish to register, please check your PIN and bring your My Number Card.
Cancellation of Health Insurance Card Usage Registration for My Number Card
If you are already registered and wish to cancel your registration, please fill out the Application Form below and submit it to the City Hall or Branch Office.
If the applicant is different from the insured person, a separate power of attorney (any format is acceptable) is required, so please submit it together.
Benefits of My Health Insurance Card
- If medical expenses become high, procedures such as the application for the ceiling amount application certificate are not required.
- Medical institutions can also check past medication information and health checkup results, allowing for better medical care.
