Eligibility
Those who have a resident registration in Inagi City on the day of vaccination and have difficulty getting vaccinated at a designated medical institution in the city for any of the following reasons.
- When vaccination by a family doctor is necessary due to medical circumstances or a doctor's judgment
- If you stay in another ward or municipality due to childbirth, illness, etc. and need to be vaccinated at a medical institution in your stay
- If you are in a medical institution, welfare facility, etc. and it is difficult to get vaccinated at a designated medical institution
- In addition, when the mayor deems it appropriate to inoculate at a medical institution other than the implementing medical institution

1 Application for issuance of “Vaccination Request Form”

3 Vaccination at a medical institution
