After April 1, 2024, the installation of the vaccination certificate app, the issuance of vaccination certificates through the app, and the issuance of vaccination certificates at convenience stores will no longer be possible.

For the paper vaccination certificate, please continue to check the following "Application for the COVID-19 Vaccination Certificate (Paper)" and submit your application.
In addition to the certificates for overseas use that have been accepted so far, you will also be able to apply for vaccination certificates for use within Japan. Furthermore, for use within Japan, the "Vaccination Completion Certificate" and "Vaccination Record" can still be used as before. No reapplication is necessary.

Application for COVID-19 Vaccination Certificate (Paper)

Eligible Individuals

Vaccination Certificate for Use Abroad

Those who have received the COVID-19 vaccine using the vaccination voucher issued by Inagi City and require a certificate for overseas travel
Please note that one certificate can be used for both overseas and domestic purposes in Japan.

Vaccination certificate for use only within Japan

Those who have received the COVID-19 vaccine using the vaccination ticket issued by Inagi City and require a vaccination certificate
Note: For use within Japan, the "Vaccination Completion Certificate" and "Vaccination Record" can still be used as before, so no application is necessary.

Issuance Method

The certificate will be issued and mailed within about one week from the date of acceptance.

Application Form

Required Documents

  • Application Form
  • Copy of passport (for overseas certificate applicants only)
  • A copy of the identification document with the return address (driver's license, Health Insurance Card, etc.)
  • A copy of something that shows the vaccination ticket number (vaccination ticket or pre-vaccination questionnaire, etc.)
  • Copy of vaccination certificate or vaccination record
  • Reply envelope (with address written and stamp affixed)

Necessary documents depending on the case

  • Documents for confirming maiden name, different surname, or alias (if the passport includes the maiden name, different surname, or alias, documents for confirming the maiden name, different surname, or alias)
  • Power of Attorney for Claims by an Agent

Note: A power of attorney is required if someone other than the applicant is applying.
