Regarding electronic application for notifications related to fire prevention field

Updated: April 16, 2024

With the Myna Portal Perfect Service, notifications related to fire prevention can be submitted electronically.

You can apply electronically from your home or work computer or smartphone.
Reports can be submitted 24 hours a day, 365 days a year.

Note: When applying, depending on the amount of attached document data, it may not be possible to attach due to system reasons. In that case, please prepare and submit two copies of various notification forms to the Fire Department Prevention Division.

Notification start date regarding fire prevention field

February 1, 2020

Target procedure

1. Fire prevention plan creation (change) notification form
2. Fire prevention/disaster prevention manager appointment (dismissal) notification form
3. Notification form for the creation (change) of a firefighting plan for the whole
Four. General fire prevention/disaster prevention manager appointment (dismissal) notification form
Five. Firefighting equipment, etc. (special firefighting equipment, etc.) installation notification form (limited to those based on Article 17-3-2 of the Fire Service Act)

Addition of applicable procedures (operation start date: February 10, 2020)

6. Firefighting equipment, etc. (special firefighting equipment, etc.) installation notification form (based on Article 58-3 of the Inagi City Fire Prevention Ordinance)
7. Self-defense fire training notification
8. Residential fire alarm installation notification form

Addition of applicable procedures (operation start date: May 1, 2020)

9. Notification of change in authority to manage fire prevention and disaster prevention management objects

11. Disaster prevention management inspection results report
12. Self-defense fire department establishment (change) notification
13. Firefighting equipment, etc. (special firefighting equipment, etc.) inspection result report

Addition of applicable procedures (operation start date: July 1, 2020)

16. Notification of appointment and dismissal of hazardous materials safety supervisor
17. Application for special certification for inspection reports of fire prevention and disaster prevention objects
18. Notification of installation (change) of equipment that uses fire (furnaces, kitchen equipment, hot air heaters, boilers, oil-fired water boiling equipment, drying equipment, sauna equipment, heat pump air conditioners, equipment that produces sparks)
19. Fuel Cell Power Generation Facility Installation (Change) Notification Form
20. Notification of installation (change) of electric discharge machine
21. Notification of Installation (Change) of Electrical Equipment (Transformation Equipment, Rapid Charging Equipment, Internal Combustion Engine-Powered Power Generation Equipment, Neon Lighting Equipment)
22. Notification of installation plan for firefighting equipment (special firefighting equipment, etc.)

Addition of applicable procedures (Start date: April 20, 2024)

23. Notification of construction plans for fire prevention facilities

Regarding handling of duplicate copies

Copies of electronic applications will not be returned.
The procedure completion email from the Fire Department Prevention Division and the copy of the application form will serve as duplicates, so please be sure to download them and keep them together with the attached documents.
Please note that you can only download the "Application Form Copy" immediately after applying.

We will update the next procedure as soon as it is ready.
