Requests for disclosure of personal information can be made by submitting a request form for disclosure of retained (specific) personal information at the information disclosure corner on the 5th floor of City Hall.

1. Please fill out the required information on the request form for disclosure of retained (specific) personal information and submit it to the Document Legal Affairs Division, Document Legal Affairs Section.
2. Please download and use the Request for Disclosure of Retained (Specific) Personal Information from the "Information Disclosure and Personal Information Protection Forms". It is also available at the Document Legal Affairs Division on the 5th floor of City Hall. Please note that when requesting the disclosure of personal information that includes your My Number, it is necessary to include your My Number.
3. When making a request, you need to present identification documents. Additionally, when requesting the disclosure of personal information that includes your My Number, you will also need to present documents confirming your My Number.
4. When making a request, you need to specify the administrative documents that contain your personal information as much as possible. If you have any questions, please consult the Document Legal Affairs Division in advance.
5. Please understand that requests made by phone or verbally cannot be accepted in order to protect your important information.
6. The service is free of charge, but if you wish to have copies or send them by mail, please pay the actual cost of the copies and shipping. Unless otherwise specified, we will use simple registered mail for shipping.
