Please tell me how to request a certificate regarding family register by mail.

Update date: March 17, 2022

If you would like the certificate to be issued by mail, please enclose the following four documents with your request.
(1) Postal invoice (Please download the application form from the "Family Registration Certificate" page)
(2) Fee (please pay by post office fixed amount money order) Note: Please contact us for the amount.
(3)Reply envelope (please write the address of the resident registration place and the name of the person making the request, and affix a stamp)
(4) Identification documents, etc. (copies of driver's license, health insurance card, etc. as documents that can confirm the requester and resident registration place)
