Due to the revision of nursing care fees for fiscal year 2024, from June 2024, the "Care Staff Treatment Improvement Surcharge," "Care Staff, etc. Specific Treatment Improvement Surcharge," and "Care Staff, etc. Base-up and Other Support Surcharge" (hereinafter referred to as the "current surcharges") will be consolidated into a single "Care Staff, etc. Treatment Improvement Surcharge" (hereinafter referred to as the "new surcharge").
・The "current surcharge" and "new surcharge" are collectively referred to as the "Care Staff Treatment Improvement Surcharge, etc."

Calculation requirements for the new addition (quick reference table), etc. are listed according to the calculation status of the current addition.
This is a support tool (Ministry of Health, Labour and Welfare) for businesses that currently calculate surcharges to consider the surcharge category for the new surcharges to be calculated after June 2024.

Community-based service business operators designated by Inagi City (including deemed designated community-based day care business operators)
(If a business located outside the city submits a notification of treatment improvement supplement to the local government where it is located, it is acceptable to submit the same form but with the address changed to Mayor of Inagi City.)
・Even for establishments that are calculating the current premium in fiscal year 2023, if they plan to calculate it in fiscal year 2024, they must submit a new plan for fiscal year 2024. In addition, if they plan to calculate the new premium from June 2024, they must submit a premium notification for the new premium.
・If you are a community-based service business located outside the city and have already terminated your use of the Inagi City insured person service, you do not need to submit a plan. Please submit a notice of closure of the business as soon as possible.
(1) Attached Form 2 (Treatment Improvement Plan)
The target is corporations that have 11 or more business locations and have been calculating the current surcharge since fiscal year 2023 or before.
(2) Attached Form 4 (Notification of Changes)
If there are any changes to the plan submitted when obtaining the care worker treatment improvement supplement, etc., a notification is required.
(3) Attached Form 5 (Notification of Special Circumstances)
If you lower the wage levels of care staff (excluding wage improvements due to surcharges) and then implement wage improvements in order to continue your business, you are required to file a notification.
(4) Attached Form 7 (For businesses not yet calculated the additional allowance, plans and performance reports)
This only applies to businesses that will be calculating the new care worker treatment improvement surcharge from fiscal year 2024 and that will be calculating new surcharges 3 and 4 from June 2024 onwards.
When calculating the new surcharges 1 and 2, or when changing the surcharge category during fiscal year 2024, please use Attached Form 2.
Note: Attachment Form 6 is no longer available because it is not expected to be used when obtaining new treatment supplements after June 2024.
If you are calculating a new addition, changing the calculation category, or terminating the calculation, you will need to attach the following documents and submit an addition notification.
Attachment 1-3 (April/May 2020) and Attachment 1-3-2 (from June 2020) are stored.
The deadline is two months prior to the month in which the additional payment is received.
Example: If you apply for a new surcharge starting in August 2024, it will be effective at the end of June 2024.
Submission of plan
Submission of notification of addition From April 2020, the principle will be to submit a “notification regarding addition” using the electronic application notification system. Please attach and submit the data from the "List of Systems, etc. Related to Calculation of Nursing Care Benefit Expenses."

When submitting a plan, it is not necessary to submit the materials that support the written content and the following documents to the city, but they must be properly stored and promptly submitted if requested by the city.
(1) Rules of employment stipulated in Article 89 of the Labor Standards Act.However, regulations regarding wages, retirement allowances, temporary wages, etc., regulations regarding appointment requirements and wage systems related to career path requirements, and regulations regarding salary increase mechanisms are included in the rules of employment. If they have been created separately, also include those regulations.
(2) Documents that confirm that you are enrolled in labor insurance, such as notification of establishment of labor insurance relationship, labor insurance estimate/final insurance premium declaration, etc.
When submitting documents, please carefully read the latest information on long-term care insurance, Q&A, explanatory materials for calculating the additional fee, and examples.
When reporting multiple business establishments at once, if a business establishment other than those under the jurisdiction of Inagi City is included, notification must also be made to the insurer that has jurisdiction over those establishments.
Businesses that have calculated the addition are required to submit a performance report. Performance reports must also be submitted by establishments that have closed down during the fiscal year.
Until the last day of the second month following the final additional payment (deposit) from the National Health Insurance Federation for each fiscal year.
(Example: If the service is provided in March, the payment is due in May, so the deadline is the end of July.)
Note: For reports for 2020, it must arrive by Wednesday, July 31, 2020.
