About the "Electronic Application Notification System" for applications for designation of nursing care facilities, etc.

The Ministry of Health, Labour and Welfare has begun accepting electronic applications and notifications for new designations, designation renewals, changes, surcharge notifications, etc. via its Electronic Application and Notification System.
Although you can still apply on paper as usual, applications will in principle be submitted via this system , so we ask that you make active use of this system.
If you are applying through the "Electronic Application Notification System," please notify the person in charge after submitting your application electronically.

About the flow of system usage

1. Creating a G Biz ID

To use the "Electronic Application Notification System", you must create a "G Biz ID" account in advance.

Note: For inquiries regarding "G Biz ID", please contact the linked inquiry form or the G Biz ID Help Desk (0570-023-797).

2. Login to the Ministry of Health, Labor and Welfare's "Electronic Application Notification System"

For information on how to operate the system, please refer to the "Operation Manual" linked below or the "Electronic Application Notification System Operation Guide (for Businesses) Explanatory Video" posted in the help section of the system.

About attached documents

For each procedure, additional documents may be required in addition to the application form. Please check the link below for required attached documents.
If you are submitting documents other than those listed on the website (copies of qualification certificates, etc.), please convert them into PDF files and attach them.
[Note] If the required documents include a "copy of the register," please submit the original separately at the counter or by mail.
