Physical disability certificate

This notebook is necessary for people with physical disabilities to receive various types of support. It will be issued when it is recognized that the disability falls under the type and degree of disability stipulated in the Physically Disabled Persons Welfare Act. The grade of the notebook is from grade 1 (most severe) to grade 6 (mild).
The areas of disability include visual impairment, hearing impairment, balance dysfunction, speech/language or mastication dysfunction, physical disability, and internal dysfunction.
In principle, there is no renewal, but if a change is expected, such as a reduction in the condition of the disability, recertification may be carried out after a certain period of time from the issuance of the certificate.

How to apply
In principle, please bring the following items with you to apply at the Disability Welfare Division counter.
(1) Application form (available at the Disability Welfare Division counter)
(2) Physical disability medical certificate/opinion (written within one year by a designated doctor as stipulated in Article 15 of the Physically Disabled Persons Welfare Act)
(3) Photo (length 4cm x width 3cm, no hat, upper body, taken within one year from the application date)
(4) Documents related to my number
Note: The application form is a photocopy type format used by the Tokyo Metropolitan Government, so it cannot be downloaded from the city website.
Note: It may be possible to accept applications for services, etc. at the same time as applying for a notebook.
In that case, proof of your bank account, a copy of your insurance card, etc. will be required depending on the service you are applying for. In addition, there are payment requirements for services, so please contact the Disability Welfare Division for details.
Physical disability medical certificate/opinion statement
Only doctors designated under Article 15 of the Welfare Law for Persons with Physical Disabilities can prepare a medical certificate or opinion for a person with a physical disability. The format varies depending on the type of disability. You can download the form from the link below.

- Based on the application, the Tokyo Metropolitan Government will conduct an examination, and if the grade is determined, a notebook will be issued. Notes may not be issued, or may be issued with a grade different from the grade listed on the medical certificate/opinion.
- It will take approximately 2 months for the notebook to be issued.
- The following procedures are required in the following cases. For details on the procedure, please contact the Disability Welfare Division.
(1) When the status of a disability changes or when a new disability is added (2) When there is a change in address or name (3) When the notebook is lost or damaged (4) When the notebook holder passes away (5) When the disability in the notebook no longer applies.
