Mental Disability Health and Welfare Handbook

This notebook is issued by prefectural governors and is necessary for persons with mental disabilities to receive various types of support. Masu).
This notebook is necessary for those who are recognized as having a state of mental disorder as stipulated in the Act on Mental Health and Welfare for Mentally Disabled Persons to receive various support systems in their daily and social lives. Grades 1 to 3 are issued depending on the state of mental illness (mental disorder) or disability. In addition, those who already have a certificate and are recognized that the degree of disability has changed can change the grade.

Application method

Please bring the following items to the disability welfare division counter or apply by mail.
(1) Application form (2) Medical certificate (Only medical certificates that are issued more than six months after the first consultation for a mental disorder are accepted. The form can also be downloaded from the website of the Tokyo Metropolitan Chubu Comprehensive Mental Health and Welfare Center.)
Note: If you are receiving a disability pension based on a mental disorder, you do not need a medical certificate if you submit a consent form along with one of the documents listed below.
・Copy of Disability Pension Certificate ・Copy of Disability Pension Decision Notice ・Copy of Disability Pension Transfer Notice (3) 1 photo (4cm x 3cm. Hatless, upper body, taken within one year from the date of application)
(4) Documents related to My Number
(5) A copy of your certificate (if renewing)
(6) Reply envelope (only if applying by mail; 110 yen stamp attached)

About application documents

If you would like us to send you the documents required for your application, please fill out the form below. It takes about two weeks from application to delivery, so if you are in a hurry, please come directly to the Disabled Welfare Division counter.

Notes on updates

 The validity period is 2 years.
 
 Renewal applications can be filed from 3 months before the expiry of the validity period.
Update notifications are sent every month via Inagi City's email distribution service.

  • Based on the application, the Tokyo Metropolitan Government will conduct an examination, and if the grade is decided, a notebook will be issued. Depending on the examination results, the certificate may not be issued, or a grade different from before renewal may be determined.
  • It takes about 3 months for the certificate to be issued.
  • When applying for medical care for independence support (psychiatric hospital) at the same time, it may be possible to apply only with the medical certificate for the notebook.
