The system is operated by a "wide-area union" established at the prefectural level.
The "Tokyo Late-Stage Elderly Medical Care Wide-Area Union," which includes all municipalities in Tokyo (62 local governments), is the operating body.
The wide-area union handles the certification (qualification) of insured individuals, determination of insurance premiums (levy), payment of medical expenses (benefits), and implementation of health projects.
City Hall is responsible for the delivery of certificates related to Late-Stage Elderly Medical Care, reception of various applications, and collection of insurance premiums.
The funding for this system comes from 50% public funds, 40% support from various insurers (such as National Health Insurance and social insurance) for those up to 74 years old, and 10% from the premiums of the insured.
