Regarding business establishment designation (home care support, community-based services, preventive care support)

1. New designation applications

If you wish to start a new business, you will need to consult in advance and submit a designated application form, etc.

(1) Advance consultation

If you wish to start a new business, please contact the person listed below by the end of the month three months prior to the month in which you plan to open your business.
(For example, if you plan to open on May 1st, please contact us by February 28th.)
Note: If you would like to start a "home care support," "community-based day care," or "dementia-friendly day care" business, please also fill out the advance notification form below and contact us.



(2) Submission of application forms for designation, etc.

After consulting with us in advance, please submit the following documents via the electronic application and notification system, by mail, or in person by the end of the month two months prior to the month in which you plan to open your facility .
(For example, if you plan to open on May 1st, please submit the necessary documents by March 31st.)



2. Application for renewal of designation

Businesses designated by the city are required to renew their designation every six years, which is the validity period of the designation.
Eligible businesses must submit the following documents via the electronic application and notification system, by mail, or in person by the 10th of the month prior to the expiration date of the validity period .




Note: Please note that the validity period of the deemed designation for home care support facilities is six years from the date of home care support designation before the amendment.
Note: Please note that if the procedure is not completed by the expiration date, the designation will become void.
Note: If you are discontinuing your business upon expiration of the validity period, please submit a discontinuation notification.

3. Procedures for making changes

If there are any changes to your application, please submit the following documents via the electronic application and notification system, by mail, or in person within 10 days of the changes .





Note: Please refer to the "List of Attached Documents (Major Items) (Common to All Services) (Changes)" for information on required attached documents.
Note: For details on submitting various surcharges, please see the "Notifications regarding systems related to calculating nursing care benefit expenses (notifications of various surcharges)" below.

4. Procedures for suspension, cancellation, refusal of designation, and resumption

If you wish to suspend or terminate a designated business or decline the designation, please submit the documents listed below via the electronic application and notification system, by mail, or in person at least one month prior to the planned date of termination, suspension, or decline of the designation.

If you wish to resume a business for which you have submitted a suspension notice, please submit the following documents via the electronic application and notification system, by mail, or in person within 10 days from the date of resuming the business .



5. Notification of the system for calculating nursing care benefits (notification of various surcharges)

If there is an acquisition or change related to an addition or deduction, please submit the "List of Systems Related to Calculating Nursing Care Benefit Expenses" and other necessary documents via the electronic application and notification system, by mail, or in person by the 15th of the month prior to the month in which the application starts.

  • Home care support facilities should check the “List of systems and other information related to the calculation of nursing care benefits” in “Appendix 1” and “Notes (1)” and then submit other necessary documents.
  • Community-based service businesses should check the “List of systems and other information related to the calculation of nursing care benefits” in “Appendix 1-3” and “Notes (1-3)” and then submit other necessary documents.

(2) Notification of new calculations or changes to classifications from April 2024

Please refer to the documents below and submit them.

6. Notification of designated nursing care prevention support entrustment (change)

If a designated care prevention support provider in this city wishes to outsource part of its designated care prevention support to a designated home care support provider, please submit the following notification form by mail, in person, or by email (koureifukushi@city.inagi.lg.jp).

After sending the email, please notify the person in charge below.
・ Notification cannot be made via the "Electronic Application Notification System."
