Family dentist referral business

We will introduce dentists from the Inagi Dental Association so that people with disabilities and those who are bedridden at home can have a family dentist in their local area.

Please fill out the application form and bring it to the health center, or apply by mail or fax.
Application forms are also distributed at health centers.
If you wish, we will send you an application form by mail, so please call the health center.
