What you need to submit
- Adoption notification (requires two adult witnesses)
- Identity verification documents of the person who came to report
- Individual number card (only for those who have the card and want to change their information. It also serves as an identity verification document.)
- Basic Resident Registration Card (Only for those who have the card and want to change their information. It also serves as an identification document.)
- When adopting a minor, a letter of permission from the family court (however, it is not necessary when adopting a direct descendant of oneself or one's spouse)
- If an adoptive parent with a spouse or the person to be adopted adopts the child alone, the consent of the spouse is required.
Possible date and time for procedures
8:30 a.m. to 5 p.m. on business days Note: 8:30 a.m. to 11:45 a.m. and 1 p.m. to 5 p.m. on public holidays. Applications will be accepted at the city hall night duty room during times other than the above. Masu.
If the office is open on holidays or at the city hall night duty room, only "deposit" will be accepted.
Note: The branch office does not accept notifications regarding family registers.
