This notebook is issued by prefectural governors and is necessary for people with mental disabilities to receive various types of support. Masu).
This notebook is necessary for those who are recognized to fall under the state of mental disorder stipulated in the Act on Mental Health and Welfare of the Mentally Disabled to receive various support systems in daily life and social life. Grades 1 to 3 are issued depending on the state of mental illness (mental disorder) or disability. Additionally, if you already have a notebook and it is recognized that the degree of your disability has changed, you can change the grade.
Please bring the following items to the disability welfare division counter or apply by mail.
(1) Application form (2) Medical certificate (Only medical certificates that are issued more than six months after the first consultation for a mental disorder are accepted. The form can also be downloaded from the website of the Tokyo Metropolitan Chubu Comprehensive Mental Health and Welfare Center.)
Note: If you are receiving a disability pension based on a mental disorder, you do not need a medical certificate if you submit a consent form along with one of the documents listed below.

The validity period is 2 years.
Renewal applications can be filed from 3 months before the expiry of the validity period.
