This is a system that subsidizes medical expenses when receiving continuous outpatient treatment for mental illness. In principle, the self-pay will be 10%. However, depending on the individual's income, income of the insured household, illness, etc., a maximum monthly co-payment amount may be set, or the entire co-payment amount may be waived.
House calls, day care, home-visit nursing, epilepsy treatment, and medication costs related to outpatient psychiatric care are also covered. Medical expenses for hospitalization are not covered.
Please bring the following items to the disability welfare division counter or apply by mail.
(1) Application form (2) Medical certificate (These can also be downloaded from the website of the Tokyo Metropolitan Chubu Comprehensive Mental Health and Welfare Center.)
Note: Prepared by a doctor within the last 3 months.
Note: This is only required every two years, so it may not be necessary when renewing.
(3) Any of the following insurance information documents (not required for those with Inagi City National Health Insurance or the Medical Care System for the Elderly)
1. A copy of the health insurance card 2. A copy of the "Qualification Verification Letter" issued by the medical insurer 3. A copy of the "Qualification Information Screen" that can be viewed on My Number Portal Note: If the patient is a member of a National Health Insurance Association (National Civil Engineering and Construction National Health Insurance Association, Tokyo Barber National Health Insurance Association, etc.), insurance information confirmation documents for all members of the household who are covered by the same insurance as the patient are required.
(4) Household income statement and consent form (5) Medical assistance for persons with disabilities (mental health outpatient) certificate (in case of renewal)
(6) Tax (exemption) certificate (this may be required if you are moving in. Please inquire in advance.)
(7) Documents that can verify the name and address of the medical institution or pharmacy (such as a patient registration card or medicine envelope)
(8) Documents related to your My Number (9) Return envelope (only if applying by mail. Attach 110 yen stamp)
If you would like us to send you the documents necessary for your application, please apply using the form below. It takes about two weeks from application to delivery, so if you are in a hurry, please come directly to the Disability Welfare Division counter.
In principle, the validity period is one year.
Renewal applications can be filed from 3 months before the expiry of the validity period.
Update notifications are sent every month via Inagi City's email distribution service.
・Based on the application, an examination will be conducted in Tokyo, and if approved, an independence support medical care (psychiatry outpatient) recipient certificate will be issued.
・It will take approximately 3 months for the recipient certificate to be issued.
・Can only be used at the designated medical institution listed on the beneficiary card.
・If there is no change in the medical condition or treatment policy, an independence support medical certificate must be submitted once every two years. However, if you apply after the validity period has passed, it will be a re-application and you will be required to submit a medical certificate.
・When applying for independence support medical care (psychiatric outpatient care) and a mental disability health and welfare notebook at the same time, it may be possible to apply with only the medical certificate for the notebook.
・Procedures are required for renewal, change of medical institution, change of health insurance, change of address, change of name, reissuance due to loss or damage, etc.
・If you have a mental disability health and welfare notebook issued based on a medical certificate for your notebook, you may be able to apply by attaching a copy of your notebook instead of the medical certificate. Please contact us for more information.
