If an accident occurs while providing nursing care services
If an accident occurs while providing nursing care services, the nursing care insurance designated business operator will take appropriate measures to protect the user's life and body, and will also notify the user's family and the home care support business related to the user. It is obligatory to contact the insurance company, etc., record the circumstances of the accident and response, and report it to the insurer.
Accident reporting process
- Accident occurs
- Contact the user's family and the home care support provider contracted with the user
- Submit an accident report in writing to the Nursing Insurance Section of the Inagi City Elderly Welfare Division within approximately 5 days (however, in the case of an emergency or serious accident, such as a fatal accident, please immediately report by telephone and submit the accident report. submission)
- If the municipality where the business or facility is located and the user's insurer are other than Inagi City, also report to the municipality.
- Submit the final accident report when all relevant accident handling is completed (however, this is not necessary if the accident handling has been completed at the time of the first report).

Note 2 When submitting electronic data, please notify the following address that an accident report has occurred. (Please include the person's name, phone number, and business email address. Please do not attach the accident report to the email .) After that, we will reply with the URL and password for uploading the electronic data of the accident report, so please upload the electronic data of the accident report to the specified URL.

Target of accident report
- If a user dies or is injured in an accident during the provision of services (including accidents during medical treatment within the facility, transportation or during medical visits, and leaving the facility without permission).
- When an outbreak of food poisoning, infectious diseases (classes 1 to 5 of infectious diseases (excluding those classified as class 5 infectious diseases subject to designated surveillance) in the "Law on Prevention of Infectious Diseases and Medical Care for Patients with Infectious Diseases," new strains of influenza, designated infectious diseases, and new infectious diseases) is confirmed
- When the provision of services is affected due to disaster damage or mechanical failure at the business premises
- If occurrence of pica, aspiration, wrong medication, etc. is observed.
- When the treatment of users is affected by illegal acts or scandals by employees (loss of personal information, loss of deposit, etc.)
- Other cases deemed necessary
Note: Please report regardless of whether the business operator is at fault or responsible.
Items that do not require an accident report
- In the case of relatively minor injuries such as scrapes or bruises
- In case of natural death due to old age or death from disease
Note: Even in the above cases, a report is required if Inagi City requests a report.
Report matters
As per the attached accident report (Ministry of Health, Labor and Welfare notification dated March 19, 2021 (Nursing Insurance Latest Information Vol. 943), "About reporting format for accidents at nursing care insurance facilities, etc."), a new reporting format was shown. Accordingly, Inagi City has also created a new format based on the accident reporting format recommended by the national government, so please download it from the link below and use it. Please make sure that the following items are not omitted : "Person in charge", "Contact information", "Medical institution location", and "Administrator name" .
Note 1: Please fill in all items in the report, including those that have not yet been determined.
Note 2 Regarding "Future measures to prevent recurrence," please discuss recurrence prevention at a meeting attended by the facility manager or manager of the business facility, and write down the results of the discussion.
Note 3 Please fill in so that there are no blank spaces.

Overview of accident report collection for FY2021
- The number of reported accidents in FY2021 was 195, an increase of 59 from the previous year .
- The top reports by service type were "paid nursing homes" with 52 (an increase of 3 from the previous year), "special nursing homes" with 40 (an increase of 17), and "dementia group homes" with 34. (up 19 cases).
- The top reported causes of accidents were 94 cases (46 cases increase from the previous year) due to physical disability, 55 cases (46 cases increase from the previous year), 55 cases (12 cases increase from the previous year), and ``Infectious diseases, etc. '' was 31 (an increase of 24).
- Of the 55 cases of ``staff carelessness, etc.,'' 33 cases were ``wrong medication/omitted medication,'' and 31 cases of ``infectious disease, etc.'' were all coronavirus infections.
Regarding medication errors and missed doses
The number of reports of medication errors and missed doses is increasing. The main reasons are lack of confirmation by staff, assumptions, and impatience due to overlap with other duties. In the worst-case scenario, incorrect medication or omission of medication can lead to death. Please make every effort to prevent accidents, and if an accident occurs, please be sure to take measures to prevent it from happening again at a meeting attended by the facility manager or manager of your workplace.
Please refer to the following as a good example of measures to prevent recurrence.
- Do not leave the patient until confirmation of swallowing is complete.
- Arrangements have been made so that multiple staff members can double-check the date and timing of medication intake, such as morning and evening.
- When confirming the type of medication, do not do it verbally, use instructions, etc., and if the problem cannot be resolved, check with the nurse or facility director.
- Changed work assignments so that they are handled by relatively fatigue-free employees who have just arrived at work, rather than those who have just left for work.
- The checklist should be filled out for each user each time, rather than all at once.
- Two staff members will check each medication before and after administering the medication, creating a system that allows for double-checking and cross-checking.
- Staff members who are administering medication should not provide any other assistance, and if other assistance is required, they should request assistance from other staff members.
