Special disability allowance

Update date: April 1, 2024

Target audience

People aged 20 years or older who live at home and require constant special care in their daily lives due to extremely severe mental or physical disabilities.

Amount paid

Monthly fee: 28,840 yen (revised from April 2020)
Payment will be made for the three months up to the previous month around the 10th day of May, August, November, and February (there are some exceptions, such as those who have been certified for the first time or those who have lost their qualifications).

Degree of disability covered

Those who fall under any of the following 1 to 4.

  1. Those who have two or more of the following disabilities (1) to (7)
  2. Those who have one disability specified in (1) to (7) below, and two other disabilities that are equivalent to class 2 disability basic pension, for a total of three disabilities.
  3. Those who have one of the following disabilities specified in (3) to (5), which is especially severe, and whose ability to perform activities of daily living is evaluated as extremely severe.
  4. Those who have one of the disabilities specified in (6) to (7) below, and whose condition is absolute bed rest or mental disability, and whose daily living ability is evaluated as extremely severe.

(1) The following visual impairments:

  1. Visual acuity in both eyes is 0.03 or less
  2. Visual acuity in one eye is 0.04 and visual acuity in the other eye is less than manual valve.
  3. As a result of measurement using a Goldmann perimeter, the sum of the peripheral visual angles of both eyes using the I/4 optotype is 80 degrees or less, and the central visual angle of both eyes using the I/2 optotype is 28 degrees or less.
  4. Those who have a binocular open visual field score of 70 points or less and a binocular central visual field score of 20 points or less as a result of measurement using an automated visual field analyzer

(2) Those with a hearing loss of 100 decibels or more in both ears. (3) Those with a severe impairment in the function of both upper limbs, or those who are missing all fingers of both upper limbs, or those with a severe impairment in the function of all fingers of both upper limbs. (4) Those with a severe impairment in the function of both lower limbs, or those who are missing both lower limbs at the ankle joint or above. (5) Those with a severe impairment in the function of the trunk to the extent that they are unable to sit or stand. (6) In addition to those listed in the preceding items, those who have a physical impairment or a medical condition requiring long-term rest that is deemed to be at least as severe as those listed in the preceding items and that makes it impossible to carry out daily activities. (7) Those with a mental disability that is deemed to be at least as severe as those listed in the preceding items.

Payment restrictions

Those who fall under any of the following cannot apply or receive benefits. For more information, please contact the Disability Welfare Division.

  1. Those whose income exceeds the income limit (Note)
  2. Persons admitted to a facility
  3. Those who have been hospitalized in a hospital or clinic for more than 3 months

Note: Whether or not the income limit is exceeded is determined based on the amount of deductions made from the income of the person himself/herself, spouse, or dependent. For details, please see the income limit table and deduction table below.

If the following conditions apply, you can receive an additional limit.

The above items are deducted from the income of the individual or spouse/support obligor to determine income.

The documents required for application are as follows. Forms for each document are available at the Disability Welfare Division.

2. Special Disability Allowance Income Status Report
3. Special Disability Allowance Certification Medical Certificate (designated form)
4. Request for direct debit of special disability allowance etc. (enter the account number in your name)
Note : Those receiving the Severely Disabled Persons Allowance do not need a medical certificate.
Note : There are fields 1 and 2 where you need to enter your personal identification number (My Number).

5. Application for dependent relatives aged 16 to 19 eligible for deduction
6. Copy of the beneficiary's pension certificate (if receiving disability or survivor pension)
7. (Non-)taxable certificate
Note : The (non-)tax certificate can be omitted if you fill in your personal identification number (My Number) in fields 1 and 2.
