Regarding the handling of home support for disability welfare services (employment services)

In light of the fact that in the remuneration revision in FY2021, home support is now handled as a regular service, regarding the handling of home support in the provision of work transition support, work continuation support type A, and work continuation support type B services in Inagi City. , as shown below and in the attached sheet. Please confirm this before implementing it.
 

Prerequisites for rewarding home support

 Note: In both cases, after prior notification to the city, it is limited to cases where it is recognized that there is a support effect equivalent to the outpatient service

(1) When the user and his or her family voluntarily request in-home support; (2) Based on the characteristics of the user's disability and the content of training provided by the business, in-home support is not suitable for the user; If training is considered effective
