Regarding malpractice claims

Update date: April 1, 2024

Regarding malpractice claims in nursing care insurance

If there is an error in the claim for long-term care benefits and comprehensive business expenses that have already been determined in the review by the National Health Insurance Association (hereinafter referred to as the National Health Insurance Federation), you will be required to submit an error petition.

Submission deadline

20th of every month (must arrive)
(Note) If the 20th falls on a Saturday, Sunday, or holiday, the due date will be the immediately preceding weekday. Applications submitted after the deadline will be treated as submissions for the following month.

How to submit

Please bring the malpractice complaint form to the counter or mail it (fax, e-mail, etc. are not accepted).

Where to submit

206-8601
2111 Higashi-Naganuma, Inagi-shi, Tokyo
Inagi City Welfare Department Elderly Welfare Division Nursing Care Insurance Section

Notes

  • We will only accept claims of malpractice that have been reviewed by the National Health Insurance Federation. If the claim is not finalized, such as for services provided in the previous month or refunds, you cannot make a mistake, so please check your track record before filing a claim.
  • If there are errors that require a re-claim, please re-re-claim the correct amount to the National Health Insurance Federation by the 10th of the month following the month in which the claim was filed (same-month error). If you cancel the achievement in full, there is no need to re-invoice. (Note) If you make an error in the same month, the difference between the canceled amount and the correct amount will be adjusted.
  • If there are a large number of malpractice claims, please contact the department in charge in advance so that we can adjust the documents and method of submission.
